Alzheimer's Association, Care Navigation
|
Care Navigators are licensed social workers who meet with individuals who are affected by Alzheimer’s disease or related dementias. We typically meet with caregivers, but can also meet with individuals who are diagnosed within the early stages of the disease. Care Navigators assist clients in identifying priority areas of concern, problem-solving, and developing a care plan. Sessions are based on a solutions-focused approach, and typically range from 1-3 appointments.

Faith in Action is an interfaith volunteer care giving initiative of the Robert Wood Johnson Foundation. Local Faith in Action programs bring together volunteers from many faith to work together to care for their neighbors who have long-term health needs.
Faith in Action volunteers come from churches, synagogues, mosques and other houses of worship, as well as the community at large. Volunteers help those in need by providing non-medical assistance with tasks such as:
